NORTHERN SKY KENNELS HEALTH GUARANTEE Northern Sky Kennels guarantees this puppy is a purebred ___________________________________ in good health and with no evidence of disease. The puppy has been checked by a licensed Veterinarian, been dewormed and has had it's first set of shots. This is a two (2) year guarantee covering genetic problems relating to: 1. HIP & ELBOW DYSPLASIA 2. HEART 3. LUNGS 4. KIDNEYS 5. BLINDNESS 6. DEAFNESS In the event your puppy is diagnosed with a genetic problem covered in our guarantee we must have documentation by a licensed Veterinarian of any and all tests, bloodwork, and xrays done to come to this diagnosis. This information must be sent to our Veterinarian (address and phone number below) along with the name and telephone number of your Veterinarian. We guarantee your pup will be evaluated with no less than a "FAIR" by the Orthopedic Foundation for Animals (OFA). Any and all claims of Hip and/or Elbow Dysplasia must have current radiographs and Veterinarian reports along with a current evaluation report by OFA. All veterinary reports, radiographs and any other test results, including the OFA report, must be mailed or faxed to our Veterinarian by a licensed Veterinarian with a contact name and phone number. No refund will be made for a dead puppy unless accompanied by a licensed Veterinarian's necropsy report stating that said pup died from a fatal genetic defect covered in our health guarantee. Our guarantee does not cover any fleas, parasites, accidents or communicable diseases in pups that have been shipped to buyer. Our guarantee does not cover any health conditions or issues brought about through the buyers neglect. To file a claim of any kind, all terms under "Care Criteria" MUST be followed and all Veterinarian visits must be documented. After you've provided us with the necessary information and data our Veterinarian will make a determination of responsibility. When the responsibility is ours you will have the option of: · Returning the puppy (at buyers expense) for a full refund of your purchase price. · Keeping the puppy and get ½ of your purchase price refunded. Should you decide to keep the pup we will need proof of the pup having been spayed or neutered. CARE CRITERIA Pup must be checked by a licensed Veterinarian within three (3) days of receiving pup. Pup must be fed Diamond Puppy Food, if you can't find this in your area, you must use Purina Puppy Chow. Pup must have NO contact with other dogs or areas other dogs may have been until pup has been checked by a Veterinarian and had it's second set of shots. No walks longer than ½ mile before 4 months of age, no more than 1 mile from 4 to 12 months. NO RUNNING WITH YOU UNTIL PUP IS 1 YEAR OF AGE. All health checks, shots and deworming must be done by a licensed Veterinarian. Pup must have a health check by a licensed Veterinarian at least every 4 months until 1 year of age, semi-annually thereafter. Pup must not be bred before 2 years of age. Pup must be fed and watered regularly and due care must be given to the length of time the pup is left in the heat, cold or inclement weather of any kind. Pups are like babies, they require a lot of care and attention. You must be willing and able to give it to them, the rewards will make all your efforts worthwhile.
